Purpose
This expression of interest form has been created for the purpose of capturing essential information about you and your professional experience to enable staff recruitment to the Wessex COVID-19 Vaccine Hub.
By completing this expression of interest form, you give consent for the NIHR Clinical Research Network (CRN) Wessex and its partners* to collect and store the information provided for the purpose of managing and coordinating the recruitment of staff.
We may also use the data you provide to support the operational running of the Wessex COVID-19 Vaccine Hub. For example, inviting you to complete a survey about your experience of working with us in order to make improvements to the registration, induction and training process.
This data will be processed with your consent.

Storing your data
Data will be stored securely in accordance with our policies for a period of three years, after which it will be destroyed.
CRN Wessex will not share your data outside of the European Economic Area (EEA), however the services that we use to process this data, including Google Apps for Business and Microsoft Forms may use servers which are located in other countries.
