Please read the following information about how we will collect and store your data before proceeding.
Purpose
You have been asked to complete this Participant in Research Experience Survey because you have previously or are currently taking part in research supported by the National Institute for Health and Care Research (NIHR).
By completing this survey, you give consent for the NIHR Clinical Research Network (CRN) Wessex to collect and store the information provided for the purpose of understanding and improving the delivery of research studies.
Your answers will be shared anonymously with local research delivery teams and the NIHR. Any information that could potentially identify you will be removed from published results.
Taking part in the survey is voluntary, you do not have to fill this form in.
This data will be processed with your consent.

Storing your data
Data will be stored securely in accordance with our policies for a period of two years, after which it will be destroyed.
CRN Wessex will not share your data outside of the European Economic Area (EEA), however the services that we use to process this data, including Google Apps for Business and SurveyMonkey may use servers which are located in other countries.
In these cases, the provider’s terms of use or the contract we hold with them will include the European Commission's standard contractual clauses or equivalent language to the same effect.
Third parties
We may communicate your information to third parties who are authorised and/or contracted to provide our services. Any such third parties must handle your information in compliance with our privacy policies.
